purpose of Clinical Trials and the associated drug registration process.
This information is held in an unidentified form in a secure database.
Boehringer Ingelheim Australia may also collect health information or
information about a person's racial background and religious beliefs as a
result of customer complaints, enquiries or an adverse reaction to a product.
This information is held in a secure database or in hardcopy in a secure
office.
It is necessary to collect information regarding a person's ethnic
background because there are genetic differences in reactions to medication
and the incidence of certain diseases. Information relating to a person's
religious beliefs is usually collected as a result of the individual
contacting Boehringer Ingelheim Australia, identifying their religious
affiliation and the fact that they are not allowed to consume certain
ingredients that may be contained in one of our products. |

Ingelheim Australia from the reporting of an adverse drug reaction is
disclosed to the Therapeutic Goods Administration (TGA). It is a condition
of registration of drugs in Australia that all adverse events are reported
to the TGA. Information collected from adverse events and customer
complaints is sent to Boehringer Ingelheim offices overseas for the purpose
of ensuring product safety, efficacy and quality. This information is only
disclosed with the individual's consent. |

| Access to Personal Information |

Individuals may access their personal information
held by BIAU by completing a Request for Access to Information Form. This
form can be obtained from the Privacy Officer. BIAU will not charge a fee
for the lodgment of a Request, however fees may be incurred for the
provision of information. Fees charged for access to personal information
will be based on:
the staff costs involved in locating and collating information;
reproduction costs,
costs incurred when bringing files back from archive
the cost of staff time in preparing and explaining the information
provided.

| Refusing Access to Personal Information |

BIAU may refuse an individual's access to their
personal information under the following circumstances:
in the case of personal information other than health information, if
providing access would pose a serious and imminent threat to the life or
health of any individual;
in the case of health information, if providing access would pose a
serious threat to the life or health of any individual;
if providing access would have an unreasonable impact upon the privacy of
other individuals;
if the request for access is frivolous or vexatious;
if the information relates to existing or anticipated legal proceedings
between the individual and BIAU and the information would not be accessible
by the process of discovery;
if providing access would reveal the intentions of BIAU in relation to
negotiations with the individual in such a way as to prejudice those
negotiations;
if providing access would reveal evaluative information generated within
BIAU in connection with a commercially sensitive decision-making process;
as otherwise allowed by the Privacy Amendment Act 2000. |

| Correcting Personal Information |

BIAU will take reasonable steps to correct
information about an individual where that information is not accurate,
up-to-date or correct. Individuals wishing to have information corrected
should complete a Request for Correction to Personal Records Form. This form
can be obtained from the Privacy Officer. No charges will be incurred for
the correction of records.
It is the responsibility of the individual seeking the correction to
demonstrate that the information held by BIAU is not accurate, up-to-date or
complete. Information will not be changed where this cannot be demonstrated.
Instead BIAU will attach a notice to the individual's personal information
outlining the nature of the individual's claim that the information was
incorrect, out of date or incomplete. |
